If you are asked for additional documentation to verify the chapter address, any of the following documents listed below are acceptable. Any document with the address of the chapter should also match the "Business Information" section of the Beneficial Ownership Disclosure Form (BODF).
- Tax Document (SS4 or 147C)
- Note: A tax document (SS4 or 147C) is the most commonly used document; however, if the address on the SS4 is different than where your chapter is located, we need another form of documentation.
- Bank Statement
- Utility Bill
- Chapter Statement
